Dopexamine

[PHW2:p199]

Structure

[PHW2:p199]

Synthetic analogue of dopamine

Pharmacodynamics

Mechanism of action

[PHW2:p199]

Stimulate beta2 receptors and dopamine D1 receptor

Minimal stimulatory effects on beta 1 receptors and dopamine D2 receptors

No effect on alpha adrenergic receptors

May also cause inhibition of NE reuptake

Effects

[PHW2:p199]

CVS

Positive inotropic effects (beta2 effect)

Reduced afterload (beta2 effect) (?due to decreased SVR)

Increased cardiac output

BP may be reduced

HR change is variable

Small increase in coronary perfusion

Rarely precipitates arrhythmia

Respiratory

Bronchodilation (beta2 stimulation)

Splanchnic

Mesenteric and renal perfusion increased

Urine output increased

Others

N&V

Tremour

Headache

Pharmacokinetics

Rapid clearance

Halflife = 7 min

Pharmaceutics

[PHW2:p199]

50mg in 5mL (pH 2.5)

Clinical

[PHW2:p199]

Used to improve CO and mesenteric perfusion

Dose range = 0.5 - 6 microgram/kg/min
